On December 18th, the world lost the "first lady of Star Trek," otherwise known as Majel Barrett Roddenberry. From what I've read at www.roddenberry.com, she died in her sleep alittle after midnight, at her home.
Although I could have turned this into a trivia question, I won't, out of respect.. Her introduction on Star Trek was in the form of playing the female character called Number One in the first Original Series pilot episode "The Cage". Afterward, she went on to play Nurse Christine Chapel throughout the Original Series as well as in the feature films Star Trek: The Motion Picture and Star Trek IV: The Voyage Home. She also interestingly lent her voice to be for the computer on the Enterprise during the Original Series, although that was not easy to tell.
In the animated Star Trek series, Majel Barrett Roddenberry not only did the voice of her Sick Bay character of Nurse Chapel, but also portrayed a wide variety of other female characters. I believe that she also did the voice of the Enterprise computer, at least in the episode "The Practical Joker", and perhaps others.
Perhaps more recognizable, she played Betazoid Ambassador Lwaxana Troi, the rather outspoken and sometimes overbearing mother of Enterprise-D counselor Deanna Troi, in several episodes of Star Trek: The Next Generation as well as in Star Trek: Deep Space Nine.
Her voice can also be heard as the Federation computer voice in an overwhelming majority of The Next Generation episodes and in all episodes of Deep Space Nine and Voyager.
In the ENTERPRISE episode "In a Mirror, Darkly" Part 2, viewers heard her as the computer voice of the Constitution-class USS Defiant.
She also was the voice of the Enterprise computer in the motion pictures Star Trek: Generations, Star Trek: First Contact, Star Trek: Insurrection, and Star Trek: Nemesis.
Star Trek game fans got to hear her voice for the Federation computer for the Star Trek TNG Interactive Technical Manual, as well as for the game Star Trek: The Next Generation - A Final Unity. Her voice can also be heard in the virtual tour program called Star Trek: Captain's Chair. I want to think there are other game credits, but I could be wrong.
One other credit that I can think of at the moment is at least one appearance in the Gene Roddenberry series called "Earth: Final Conflict".
Star Trek fans will get the chance to hear her voice again as the Federation computer in the upcoming J.J. Abrams-directed Star Trek movie, due out in theaters in May of 2009.

Perhaps one of my absolute favorite Majel Barrett moments in Star Trek: The Next Generation occurred in the episode "Cost of Living". Fans may recall this episode as the one where Ambassador Lwaxana Troi was on the Enterprise to be married to a man she had never met face-to-face.
In-character as Lwaxana Troi, she went to the holodeck to run a holodeck program with her 'little warrior' Alexander Rozshenko in tow. Viewers got to hear her talents at playing two characters simultaneously when Majel Barrett as Lwaxana Troi was speaking with Majel Barrett as the Enterprise computer voice.
I do believe there was another moment like that in the second season of TNG, in the episode "Manhunt".
