That would depend on your interpretation and whether you consider the non cannon sources. It has been suggested, by Peter David in Q-Squared for example, that Trelane (From The Squire of Gothos) was actually a young member of the Q Continuum. If you consider him a Q , then the answer would be William Campbell.

1 - Seven of Nine.
The episode featured a race that had more-or-less a xenophobic attitude toward holograms, referring to them as "photonics" with disgust..
2 - B'Elanna Torres.
In "The Killing Game", the holodeck character that B'Elanna was playing was evidently pregnant already, and so Roxann Dawson's natural pregnancy was allowed to be seen, and it would be noticeable up through the Season 4 finale "Hope and Fear"
