Registration
Ship Class
Where Ship Was Built (Default: Utopia Planitia, Mars, Sol)
Year Launched (Or Stardate)
Special Infomation (First Ship Of Her Class, Third Ship To Bear The Name)
Quote ("To Boldly Go...", "Where ever you go, there you are", etc.)

Theme (Pick 1 of the following)

Gold


Silver


Wood


Marble
